HIM Forms, including disclosure and consent forms, moving to new online location Sept. 9

Health Information Management is in the process of moving the Forms Repository from MyUTMB and Sharepoint to OnBase. The transition, which will consolidate HIM forms in an indexed, easier-to-search system, has already begun.

On Tuesday, Sept. 9, the HIM Sharepoint-housed files will be retired completely, and all forms will be available exclusively through OnBase.

Until then, both Sharepoint and OnBase will be available to users. You will still be able to search for and print forms just as you did in MyUTMB.

To support this change, the Health Information Management website has been updated to include links to all forms, including Disclosures and Consents, in both current locations. After Sept. 9, OnBase will be the sole source for forms.

Tunnel Access in Hospital Garage (University Plaza Garage) partially blocked Sept. 6 - 13

Work to install French drains in a section of the University Plaza Garage will begin Saturday, Sept. 6. The project should be completed and the route reopened by Sept. 13. The access tunnel to be closed is the one that connects the garage to the Administration Building.

The work will improve drainage; signs will direct pedestrians to alternate routes.

Disaster/Weather Emergencies

In the event of a disaster or emergency weather conditions, please refer to the following lines of communication for more information:

  • Call the UTMB main operator at (409) 772-1011.
  • Information is also available via the web at www.utmb.edu/alert. Should the UTMB web server go down, a second server is in place as a backup and will be available via UTMB's normal URL: www.utmb.edu.  Additionally, an informational site will be activated at www.utmbinfo.com.
  • See www.utmb.edu/emergency_plan for UTMB Health Emergency Operations Plans.
  • Monitor local television and radio stations including KPRC/950 AM, KTRH/740 AM and KIKK/96 FM in Houston.




UTMB Alerts is a "reverse 911" system, the application augments existing emergency communication resources such as email, web and phone-based alerts. Stay informed.
